In compliance with the No Surprises Act (you can find more information here), AP Counseling and Yoga will provide all new clients with a No Surprises Act Consent Form and Good Faith Estimate for treatment with their new client intake paperwork. 

  For all current clients annually we will be collecting an updated No Surprises Act Consent Form and Good Faith Estimate. If you would like to view our No Surprises Act consent form you do so here.
